Remote Monitoring by CC requires that your healthcare provider is enrolled in the Remote Monitoring by CC platform. Please have your healthcare provider [email protected] to enroll.

Notice: The Remote Monitoring by CC app supports Apple HealthKit which integrates with the Health app.
